Social Worker(24/7)

At Centric Care Hospice, our Social Workers play an important role in providing compassionate care to our patients and their families. They are trained professionals who offer counseling and support to individuals and families facing difficult end-of-life decisions. Our Social Workers are dedicated to helping patients and families navigate the healthcare system and understand the various services available to them.

Our Social Workers are available to address questions regarding hospice services, financial assistance, Veteran programs, and other community resources. They are also responsible for making arrangements for respite admissions and contacting community resources as needed.

In addition to their administrative responsibilities, our Social Workers provide counseling and supportive services to patients and families. They offer bereavement support to help individuals cope with the loss of a loved one and provide resources for community services to help patients and families meet their needs.

Our Social Workers work closely with our interdisciplinary team of healthcare professionals to ensure that each patient receives the highest quality of care. They are committed to providing compassionate care to our patients and their families and strive to make the hospice journey as comfortable and peaceful as possible.
